The chapter on ultrasound, which is one of the most important imaging techniques required in ART, is detailed with follicular monitoring and Doppler studies and its implications in ART. Pictures to show oocyte retrieval, embryo transfer, and IUI are self-explanatory about the methods and the difficulties that can occur. The chapter on sperm retrieval techniques is exhaustive and shows detailed techniques of TESA and Microtese. There are chapters dedicated to laboratory work with respect to the sperm, oocyte, and embryo. Aspects of semen analysis, sperm morphology, sperm DNA damage assessment, and sperm preparation methods have been covered in detail through pictures. Retrograde ejaculation has been discussed with the clinical and laboratory aspects in mind. There is no better way of depicting oocyte and embryo morphology and grading than pictures and this is well represented in the Atlas. Traditional farm animal species are routinely included in veterinary curricula, have immense volumes of published information, and most large animal veterinarians and veterinary technicians possess experience and a general understanding of procedures to be performed on these species. This wealth of resources greatly outweighs that available regarding South American camelids for most veterinarians. South American camelids are increasingly ingrained in modern veterinary culture, but this experience is relatively new—measured in decades versus centuries for that of traditional species. Early on, medical and surgical care was directed toward specialty clinics and veterinarians with special interest. This was largely due to the extreme monetary value of animals and veterinarians’ apprehension to treat animals in which they have little to no experience. Also, many practitioners have limited direct practice experience with llamas and alpacas. Many methods for common procedures as well as specialized investigative procedures have been developed through trial and error. The general acceptance of South American camelids as a common component of mixed animal veterinary practice has led to veterinarians finding themselves performing examinations, diagnostic testing, and medical and surgical procedures on llamas and alpacas. Comfort working with the species has grown as graduating veterinarians receive more formal training and experience during their education.
We receive and address many calls from the field and from other academic institutions on how to perform varying procedures. Our goal with this text is to synthesize our combined experience with procedures from routine maintenance to advanced procedures into a single, organized, concise, visually descriptive volume for veterinary reference. Integrative Veterinary Medicine (IVM) is a new clinical approach to veterinary medical care that combines conventional (Western) veterinary medicine, as traditionally taught in veterinary schools, with complementary therapies to improve clinical outcomes in patients. The idea for this book stemmed from a publication, co-authored by 26 board-certified veterinarians, which proposed curriculum guidelines for veterinary schools to introduce students to IVM (Open Vet J 6:44–56, 2016). Topics suggested by the publication have been expanded in this book and detailed information is covered in 26 chapters. All chapter authors are talented integrative clinicians with expertise that include one or more AVMA-recognized specializations and/or certification in multiple areas of complementary therapies.

Aging seems to be a longer lasting pandemic than COVID. Older people enjoy living happily despite a rapidly increasing disease burden, and this has to do with the robust and expanding achievements of hygiene and medicine. This may or may not include the most important option for treatment in almost all therapeutic areas, which is the prescription of drugs. Medication can positively contribute to disease prevention and mitigation, but due to inevitable side effects it could also be limiting the enjoyment of life or even end it.
This topic of medication in older people thus is of paramount importance for this steadily growing population part of our industrial societies. The new book on Optimizing pharmacotherapy in older patients is one of the very few attempts to condense our knowledge on this issue and comprehensively detail the dimensions, implications, and technical accommodations for coping with this challenging aspect of medicine. In its 29 chapters, the book deals with the epidemiology of “polypharmacy” (a widely used, but incorrect term which should rather be multimedication), the detrimental impact of some pharmacodynamic and pharmacokinetic features of the aging body on therapeutic outcomes and on instruments, practices, and tools to optimize medication use in older people. Catchwords like “deprescribing” or “potentially inappropriate medications” (PIM) are defined and discussed in the current scientific context. Drug–drug, drug–nutrient, and drug–disease interactions are being exemplified, issues of adherence and the roles of potential networking partners such as pharmacists addressed in chapters by outstanding researchers in the area.
The content of this first section on general issues is applied to geriatric syndromes such as frailty or falls, and to special aspects of medications for frequent diseases in older people (e.g., hypertension or heart failure) in the second main part of the book.

With a lot of data being generated on the Human Microbiome, quite many correlations are being uncovered between the microbiome composition and human health/ diseases. Almost every aspect of human health and behavior, e.g., obesity, functioning of the immune system and nervous system, susceptibility to various infections, and mood fluctuations, seems to be correlated with the microbiome composition. Although the microbiomes of the skin, oral cavity, nasal cavity, and vagina have their own importance, much research has focused on the microbiome of the intestine since the gut harbors the greatest number of resident microorganisms within the human body. As we are developing more and more understanding of the human microbiome and its role in the regulation of the human system, more interest is being generated in finding ways of manipulating this microbiome toward the “healthier” direction. This manipulation of the human microbiome seems to be possible through the use of the following:
• Probiotics: Live microbial strains which, upon ingestion, are expected to confer some health benefit.
• Prebiotics: Selectively fermented ingredients causing specific changes in the composition and/or activity of the gut microbiota, thereby offering beneficial health effects to the host.
• Synbiotics: A mix of probiotics and prebiotics that can confer some beneficial effect on the host by improving the survival and activity of beneficial microorganisms in the intestine.
• Postbiotics: Metabolic products of the fermentation by probiotics in the intestine.

Since its inception three decades ago, Haschek and Rousseaux’s Handbook of Toxicologic Pathology has been a comprehensive resource covering fundamental knowledge and skills as well as key technical procedures essential for the proficient practice of toxicologic pathology. The reference has found a home in the libraries of numerous academic, government, and industrial institutions engaged in basic and applied biomedical research around the globe, and in doing so has become an indispensable reference for many toxicologic pathologists, toxicologists, regulators, physicians, biomedical researchers, and students. Indeed, the Handbook has been recognized by many as the most authoritative single source of information in the field due to the breadth and depth of coverage for this field .
Prior to publication of the inaugural Handbook edition in 1991, information regarding toxicologic pathology had to be gleaned in a piecemeal manner by reading articles in various toxicology and the few toxicologic pathology journals. This 4th edition of Haschek and Rousseaux’s Handbook of Toxicologic Pathology has been extensively updated to continue its comprehensive coverage. This update required extensive expansion, which was initially conceived as a four-volume text. However, the unrelenting explosion of information in this field instead has necessitated that this new edition instead be rendered as a five-volume set where the contents of Volume 2 of the 3rd edition now are divided between Volumes 2 (“Toxicologic Pathology in Safety Assessment”) and 3 (“Environmental Toxicologic Pathology and Selected Toxicant Classes”). Volume 3 will be the first volume or book completely dedicated to environmental pathology since the 1990s.

Volume 3 is dedicated to “Environmental Toxicologic Pathology and Selected Toxicant Classes.” This volume covers toxicologic pathology as it relates to food (Chapter 2), nutrition (Chapter 3), and selected toxicant classes in the environment that affect human and animal health (Chapters 4e14). New chapters include herbal products (Chapter 4) and animal- and bacteria-derived toxins (Chapters 8 and 9); for these chapters, particular attention has been paid to the role of such substances in formulating new therapies (either as the active agent or as important contaminants of the manufacturing process). All previous chapters (e.g., endocrine disruptors, heavy metals, nanoparticulates, poisonous plants, radiation) have been extensively revised and updated. Organspecific environmental diseases will be addressed ivolumes 4 and 5.
• Volumes 4 and 5 provide deep and broad treatment of “Target Organ Toxicity” for major systems, emphasizing the comparative and correlative aspects of normal biology and toxicant-induced dysfunction, principal methods for toxicologic pathology evaluation, and major mechanisms of toxicity. New topics addressed in Volume 4 include the toxicologic pathology of adipose tissue, tendons, and teeth. New information regarding product development and regulatory issues has been incorporated in the organ system chapters for both volumes.
The expanded coverage of nonclinical testing principles and practices, the new material on various models of toxicity, and the toxicologic pathology of novel toxicant classes (especially new therapeutic modalities) will be a particular strength of this new Handbook edition and should and will continue to justify its long-standing reputation for excellence. Since its inception three decades ago, Haschek and Rousseaux’s Handbook of Toxicologic Pathology has been a comprehensive resource covering fundamental knowledge and skills as well as key technical procedures essential for the proficient practice of toxicologic pathology. The reference has found a home in the libraries of numerous academic, government, and industrial institutions engaged in basic and applied biomedical research around the globe, and in doing so has become an indispensable reference for many toxicologic pathologists, toxicologists, regulators, physicians, biomedical researchers, and students. Indeed, the Handbook has been recognized by many as the most authoritative single source of information in the field due to the breadth and depth of coverage for this field .
Prior to publication of the inaugural Handbook edition in 1991, information regarding toxicologic pathology had to be gleaned in a piecemeal manner by reading articles in various toxicology and the few toxicologic pathology journals. The success of the one-volume 1st edition and the expanded roles of toxicologic pathologists over time led in due course to the release of updated Handbook versions in subsequent years: a two-volume 2nd edition in 2002 and a three-volume 3rd edition in 2013. The many scientific advances, ongoing and extensive collaboration among global societies of toxicologic pathology, and new regulatory guidance that has occurred since release of the 3rd edition amply prove that another rendition is necessary to maintain, and indeed enhance, the value of this resource to practitioners of the toxicologic pathology craft. This 4th edition of Haschek and Rousseaux’s Handbook of Toxicologic Pathology has been extensively updated to continue its comprehensive coverage. This update required extensive expansion, which was initially conceived as a four-volume text. However, the unrelenting explosion of information in this field instead has necessitated that this new edition instead be rendered as a five-volume set where the contents of Volume 2 of the 3rd edition now are divided between Volumes 2 (“Toxicologic Pathology in Safety Assessment”) and 3 (“Environmental Toxicologic Pathology and Selected Toxicant Classes”). Volume 3 will be the first volume or book completely dedicated to environmental pathology since the 1990s.
For the 4th edition, the new five-volume design distributes important concepts in the following way.
• Volume 1 (released in 2022) covers “Principles and the Practice of Toxicologic Pathology,” including such key topics as basic concepts in biology, pathology, pharmacology, and toxicology as they intersect in the field of toxicologic pathology (Chapters 2e8); primary methods in toxicologic pathology (Chapters 9e16); overviews of major models employed in toxicologic research (Chapters 17e24); and essential practices in generating and interpreting toxicologic pathology data (Chapters 25e29). New chapters in Volume 1 address absorption, distribution, metabolism, and excretion (ADME) principles for biomolecules; toxicologic pathology considerations in developmental and reproductive toxicity (DART) testing; digital pathology; and various animal models including rodents, rabbits, dogs, nonhuman primates, and alternative models (e.g., in silico, ex vivo). Because of the recent decision to divide Volume 2, leading to a five-volume 4th edition, chapter cross-referencing in the initially released Volume 1 (by volume and chapter number) may be incorrect when referring to contents of other volumes.
• Volume 2 (released in 2023) addresses “Safety Assessment and Toxicologic Pathology.” Safety assessment covers the application of toxicologic pathology in developing specific product classes (Chapters 1e12) and principles of data interpretation Chapters 13e17). In mid-2018, Dr. Wayne Paris stepped off the airplane as he arrived at Birmingham’s Shuttlesworth Airport in Alabama. The city of Birmingham is not all that different from Paris’ hometown and institution in Abilene, Texas, and he was excited to spend a year-long sabbatical at The University of Alabama at Birmingham (UAB) developing and advancing a familiar field of interest to him—pig-to-human organ transplantation or Xenotransplantation.
While many multidisciplinary collaborations in academia surge for various reasons, the particular relationship between the three editors of this book—who hold different training and were on various career trajectories—surged from a shared passion, intrigue, stubbornness, privilege, and joy of pursuing their professional interests. How their paths crossed and coincided into the field of xenotransplantation begins in 1979 when Paris graduated with his master’s degree in Social Work from the University of Oklahoma. Prior to moving into academia in 2004, Paris worked as a Clinical Transplant Social Worker (LCSW) for 25 years in medicine and organ transplantation at the Nazih Zuhdi Transplantation Institute at Integris Baptist Medical Center in Oklahoma City, Oklahoma. His clinical experience was primarily in heart, lung, and liver transplantation. In addition to his clinical work, Paris had an extensive clinical outcome research and publication practice, and presented at numerous national and international medical and social work conferences. He was active in several social work and medical societies and regularly served on national and international social work and medical meeting abstract and grant review committees during that time.
In 2004, Paris accepted an Assistant Professor position at the School of Social Work at Southern Illinois University Carbondale (SIUC). He taught across the curriculum in the undergraduate program from 2004 to 2010. While at SIUC, Paris completed his PhD in Social Work from the University of Huddersfield, Yorkshire, United Kingdom in 2006, a doctoral program that specialized in clinical outcomes research.
In 2010, Paris was recruited to serve as Graduate Program Director of the Masters of Science in Social Work at the School of Social Work at Abilene Christian University (Abilene, Texas). He served in that capacity for 5 years and directed one of the few accredited social work programs in the United States that requires a graduate thesis. After stepping down as Graduate Program Director in 2015, he reconnected with his mentor David K.C. Cooper, MD, PhD, FRCS. Dr. Cooper at that time was Co-Director of the Xenotransplant Program in the Department of Surgery at UAB and an internationally recognized leader in xenotransplant research who was making scientific progress in preparation for pig-to-human kidney clinical trials. Some final preparations for those trials recommended that, in addition to biological advancements, the development and conducting of psychosocial research about the public’s willingness to consider the procedure. This would be in line with recommendations from the World Health Organization, International Xenotransplantation Association, and United States Food and Drug Administration to secure local, “relevant opinions.”
Dr. Luz Padilla obtained her medical degree from the University of Guadalajara in Mexico. Her interest in disease prevention through evidence-based research led her to pursue a Master’s of Science in Public Health in Epidemiology in 2014 at UAB. She became faculty at UAB in 2016 and was appointed Director of Surgical Research of the Pediatric and Congenital Heart Center at UAB and Children’s of Alabama.
Dr. Daniel Hurst was, at the time, Clinical Assistant Professor at UAB teaching undergraduate bioethics, working in clinical ethics, and leading the research for the Cahaba-UAB Family Medicine Residency Program. Hurst’s training is multidisciplinary, holding two graduate degrees in theology, a MSc in Global Health and Infectious Diseases from The University of Edinburgh, and PhD in healthcare ethics from Duquesne University.
In early January 2019, Hurst, Padilla, and Paris gathered in Padilla’s office at UAB. Hurst and Paris had met just a few days earlier over coffee at the hospital’s Starbucks, introduced by a mutual friend who knew they had similar interests in bioethics. Gathered in Padilla’s office that day, what began to coalesce was the beginning of a multidisciplinary (medical, social, and ethical) group devoted to studying the ethical and social implications of xenotransplantation. Since then, the three editors have had multiple publications, research studies, and now this book in hopes to make a contribution and be better prepared as xenotransplantation makes its way to clinical trials. The editors feel fortunate to have important figures in xenotransplantation agree to collaborate and contribute their expertise in this book, and we hope the book, at least in some small way, can advance the field.

Hematopoietic stem cell transplantation (HSCT) is an important treatment that is increasingly offered around the world for a variety of malignant and nonmalignant conditions. The survival of HSCT recipients has been steadily improving because of advances in conditioning regimens, supportive measures, and management of complications. However, HSCT recipients continue to have issues related to pancytopenia, side effects of conditioning regimens, and GVHD and its management. These complications range from infectious to noninfectious and affect almost every organ, occasionally leading to critical illness requiring admission to the intensive care unit.
This book focuses on the spectrum of pulmonary conditions that may develop following HSCT. These conditions continue to be a major cause of mortality and morbidity in this patient population and range from infectious—including bacterial, fungal, and viral—to noninfectious—such as diffuse alveolar hemorrhage, idiopathic pneumonia syndrome, interstitial lung disease, and bronchiolitis obliterans syndrome. The chapters also provide an overview of the major complications associated with other organs that may lead to critical illness including cardiac, neurologic, gastrointestinal, and renal complications. Experts from around the world discuss risk factors, clinical presentations, and advances in the diagnosis and management of these conditions.
This book is unique in several ways. It is the first work that is solely devoted to the topic of pulmonary and critical care complications of HSCT. Physical examination techniques have been the basis for medical care for hundreds of years and have been refined over time. Evaluation of the patient and determination of a specific diagnosis is critical in determining the patient’s condition and need for treatment. We are fortunate to have developed technologies which facilitate the diagnostic process. One may be tempted to rely exclusively on diagnostic studies such as diagnostic imaging, laboratory testing, and/or invasive monitoring to reach a conclusion. The increased use of telemedicine has bolstered reliance on the results of studies instead of physical examination. This reliance on technology would suggest that physical examination of the patient is superfluous.
As technology is not infallible, the goal of this publication is to educate the reader regarding the importance of a well-performed examination during which the practitioner physically examines the patient and evaluates various characteristics of the musculoskeletal system to reach a diagnosis. Many times, there is a discrepancy between physical examination findings and diagnostic technology results such as from an MRI scan. Having the ability to critically assess the injured body part to determine if the physical findings are consistent with the MRI results is critical to determine if treatment is necessary. Therefore, we have an obligation to our patients to be able to create a differential diagnosis based on the physical examination.
